Are staff members notified of the Custom Form submission?

Staff can receive the PDF form in an automated email if an Automation is created.

You can specify different/additional staff to receive the completed forms by adding individual email addresses to the Custom Email Template for different forms.

For example:

Completed medical forms can be emailed straight to the school nurse.

Completed media consent forms can be emailed straight to the communications/marketing team etc.

Do Custom Forms accept payments?

Custom Forms can be setup to receive payments.

How do parents access Custom Forms?

Custom Forms are listed in the parents’ Enrolment Dashboard for action in their ‘Outstanding Tasks’ box. Custom Forms can be targeted at particular year levels, calendar year of entry and application statuses so you don’t have parents filling in forms that they don’t need to.

You can also send a link to a private Custom Form if you do not wish to have them on the Enrolment Dashboard.

Note: If a Custom Form is made Private it is unable to be tracked in Reports > Custom Form Submits

Where can the Custom Form data be viewed?

A copy of the completed form is appended to the relevant Student Profile for future reference.

You can also view the response on the Student Profile sub-menu tab.

Is there reporting available for Custom Forms?

In the Reports section of EnrolHQ you can view the status of each type of Custom Form. There are bulk actions that can be made for all Custom Forms. See Custom Form Submits Report

Do Custom Forms meet compliance standards?

Every Custom Form you create can be electronically signed by parents ,so all consents that you receive meet compliance standards.

Custom Forms work especially well on touch devices.

Are there other use cases?

While most schools will use Custom Forms to collect onboarding data at Offer or Orientation stage, schools will also use them throughout the student's time at school. I’ve worked with schools who ‘re-enrol’ their students every year, so those parents need to complete the re-enrolment form. It is best practice to ask all your parents to login to update Medical/Emergency Contacts/Contact Details from their Enrolment Dashboard at the beginning of the year, a task that is easy to do with Custom Forms.

One of the biggest issues schools have is actually getting current parents to update consents and emergency/contact details. Custom Forms makes it easy for them and Automations will follow up incomplete forms for you, freeing up your time to focus on more important tasks.

Can I get a PDF copy of the form submission?

Every Custom Form that is submitted by the parent can be printed in PDF, including their electronic signatures. Having a PDF copy helps schools meet another compliance requirements.

Can I export Custom Form data?

You can export all the Custom Form data as a CSV file. The form fields will form the columns of the spreadsheet and each student that has submitted a Custom Form will be displayed in a new row in the exportable file.
