What are custom forms

Build your form, add however many different questions (with their own answer types) as you want, add any introductory/explanatory text, add sections and even segment the form into multiple steps. The best part is that these forms are all connected to EnrolHQs standard workflow tools for improving school productivity and compliance. Schools have the ability to create custom forms for any additional information they wish to ask from parents. Custom forms can be used for parent consent forms, medical updates or interview documents.

Custom forms were designed to complement the enrolment application form, but the feature has a life of its own now and schools use them for all kinds of things. They are all accessible through Parent Portal and can collect a variety of information from parents. They have the nice flow indicator on the side and have the school branding on the them. Here is an example of what parent sees when filling in a custom form:

Creating Custom Forms inside the admin is an easy and intuitive process. Our custom form creator looks something like this at the time of this writing, but the look of it may change:

After the offer of place has been accepted, EnrolHQ goes one step further and you are able to build custom forms for parents to fill out and electronically sign as part of the students’ orientation. We see schools using this for things like (but not limited to)

  • Media Consent Forms
  • Medical Consent Forms for administering medication, calling ambulances/doctors in an emergency
  • IT Agreement Forms
  • Updating Medical Details/Emergency Contacts
  • Updating Learning Support
  • Additional Document Uploads
  • Co-curricular Sign-Ups
  • Bus / Travel Arrangement Forms
